Registration Forms
For 2026-2027 School Year:
- 2026 – 2027 Free and Reduced Lunch Form (One per family in any building)
- Food Allergy Action Plan (if your student has food allergies – update each year)
- Diet Modification Request Form
- Asthma Action Plan (if your student has asthma – update each year)
- Certificate of Dental Screening (ALL 9th Grade Students)
- Medical History and Permission Form (1 per student each year, both sides if student takes any medications at school)
- Heads Up Concussion Form (1 per student each year for those participating in sports)
- Sports Physical Form (1 per student each year for those participating in sports)
- Home Language Survey (1 for student file if NOT filled out last year)
- Transportation Request Form (One per family in any building)
- 2025-2026 Registration Form (High School)
- 2025-2026 Waiver Statement (High School)
- Acceptable Use Policy (1 per student each year – must be signed by student & parent before any technology use)
- Certificate of Immunization Exemption – Religious
- Certificate of Immunization Exemption – Medical
