Registration Forms – Fenton Campus
FORMS FOR ALL STUDENTS:
- Field Trip & Internet Parental Consent Form (**All students, one per family)
- Medical History and Permission Form (**1 per student each year, both sides if student takes any medications at school)
- Transportation Request Form (**One per family in any building)
- 22-23Transportation Schedule (times and locations for bus stops, will be updated after registration)
- Parent/Student/Teacher Compact (**for all students & parents to sign, grades K-5)
- 2024-2025 Free and Reduced Lunch Application (One per family in any building) If you are direct certified, no need to fill out!
NEW/PK STUDENTS or SPECIAL HEALTH NEEDS FORMS:
- Home Language Survey (1 for student file for PK & new students to building)
- Google Additional Services Use Consent Form (New students only, returning students will initial previous form grades 1-5)
- Preschool and Kindergarten Pre-Admission Medical Exam (if done for PK, a new one is not needed)
- Certificate of Dental Screening (PK & Kindergarten, if done for PK, a new one is not needed)
- Certificate of Vision Screening (for ALL Kindergarten and Third Grade Students)
- Food Allergy Action Plan (if your student has food allergies – update each year)
- Asthma Action Plan (if your student has asthma – update each year)
- Diet Modification Request Form (only if needed for food allergies)
- Elementary Technology Policies and Acceptable Use of iPads/Chromebooks Document (info only, no need to print out)
