Middle School Athletic Concussion Form
If you are participating in MS Athletics, you need to have this Form completed and submitted to the MS Office before you can participate in practice or games.

Reminder to all North Union Jr. High Sports Parents: When there are Jr. High games at the Swea City building they are considered HOME games and the students will not be bused anywhere after the game. Your child will need to be picked up from the school that is hosting the event.
When there is an away game the students will be bused back to the North Union Middle School Building in Swea City and the parents will need to pick them up there.
