Registration Forms
For 2025-2026 School Year:
- 2023-2024 Free and Reduced Lunch Application (One per family in any building)
- Food Allergy Action Plan (if your student has food allergies – update each year)
- Asthma Action Plan (if your student has asthma – update each year)
- Certificate of Dental Screening (PK & Kindergarten, if done for PK, a new one is not needed)
- Certificate of Vision Screening (for ALL Kindergarten and Third Grade Students)
- Preschool and Kindergarten Pre-Admission Medical Exam (if done for PK, a new one is not needed)
- Health Information and Consent Form (1 per student each year, both sides if student takes any medications at school)
- NU Medication Authorization
- Heads Up Concussion Form (1 per student each year for those participating in sports)
- Sports Physical Form (1 per student each year for those participating in sports)
- Home Language Survey (1 for student file if NOT filled out last year)
- Transportation Request Form (One per family in any building)
- Diet Modification Request Form
