Requirements for Entering School Preschool:  
Immunizations:

  • Immunization are required, and need to be current and age appropriate for your child. Documentation is necessary and on a Certificate of Immunization form.
  • 4 Doses of DPT (Dipthera, Pertussis, and Tetanus)
  • 3 Doses of Polio 3 Doses of Hib 1 Dose of MMR (after 12 months of age)
  • 1 Dose Varicella (chickenpox) after 12 months of age or proof of disease.
  • Prevnar Dose (pneumococcal) determine by age and vaccination schedule.
  • Need boosters after age four and before entry into kindergarten.

Physical:

  • Is now required.

Blood Lead Testing:

  • Not required. Will need to have to enter kindergarten. Best to get it done.

Dental Visit:

  • Not required, but encouraged.

Vision Screening:

  • Not required, but encouraged.

Birth certificate:

  • Required.
